API: PolyAI Outbound Calling API (proxied server-side)
Auth: Token stored in Cloudflare environment variable

Submit sends form data to /api/outbound → Cloudflare Worker proxies to PolyAI → outbound call fires to the phone number entered. Patient data is passed as metadata and stored to conv.state + agent memory.
